Texas HB 4224 – Effective September 1, 2025
In accordance with Texas Health and Safety Code Chapter 181 and House Bill 4224 (89th Regular Session), patients have the right to access their health care records and to receive information on how to submit questions or complaints.
Requesting Your Health Care Records
Patients may request copies of their medical records from our office using any of the following methods:
- In Person: Submit a written request at our front desk during normal business hours.
- Or downloading a records release here: (shortcut to form) and submitting it via the methods below:

Requests are processed in accordance with Texas law. Reasonable, legally permitted fees may apply.
